We work closely with your practice manager or nurses to identify patients who would benefit from a Home Medication Review (HMR). We’re here to make the process as smooth and effective as possible.

Eligibility

Patient’s Eligibility

Currently taking five or more regular medications

Taking more than 12 doses of medication per day

Symptoms suggestive of an adverse drug reaction

Medication with a narrow therapeutic index or medications requiring therapeutic monitoring

Significant changes made to medication treatment regimen in the last three months

Patients attending a number of different doctors, both general practitioners and specialists

Sub-optimal response to treatment with medicines

Recent discharge from a facility / hospital (in the last four weeks)

Suspected non-compliance or inability to manage medication related therapeutic devices

Patients having difficulty managing their own medicines because of literacy or language difficulties, dexterity problems or impaired sight, confusion/dementia or other cognitive difficulties

THE PROCESS

How does it work ?

Assessing and identifying patients suitable for a Domiciliary Medication Management Review (DMMR) according to the Medicare Benefits Schedule (item 900)

After completing the assessment, refer your patient to Reliance HMR’s network of Accredited Pharmacists and provide us with the necessary clinical information

Review the pharmacist's assessment and the proposed medication management strategies.

Create a written medication management plan after discussing it with the patient and send a copy to our network

Setting Expectations

What you expect from our pharmacists ?

As part of our comprehensive care approach, an HMR-accredited pharmacist will reach out directly to your patient to arrange a convenient time for an in-home visit. During this visit, the pharmacist will conduct a thorough medication review, ensuring they meet your patient in a professional and respectful manner. Following the consultation, the pharmacist will compile a detailed report, which will be forwarded to your clinic within two weeks of the patient’s visit. This report will provide valuable insights to support your ongoing care plan.
